If you wish to cancel your membership, you may visit us in person at the Center, call us at (309) 433-9355 (WELL) or fill out the cancelation form below.

Please note pursuant to our Membership Policies that all cancelations are effective on the last day of the month. Any notice, request, or submission received by the Wellness Center on or before the twentieth (20th) day of the month will become effective on the last day of that month.  Any notice, request, or submission received after the twentieth (20th) day of the month will become effective on the last day of the following month.

Cancelations, Freezes and Membership Status Change

FREEZE:

You may suspend/freeze your account in the following ways:

  • Temporary Suspension/Freeze Status.  You may apply for a “Temporary Suspension/Freeze” of your membership for a minimum period of one (1) month, up to a maximum of three (3) months, and no more often than once every calendar year by: (i) giving advance written notice to the Wellness Center; and (ii) paying all dues and other unpaid charges due at the time of the commencement of the suspension/freeze. 
  • Military Suspension/Freeze Status.  If you am leaving for active military duty or military training, you may apply for “Military Suspension/Freeze”, not to exceed twelve (12) months, with presentation of military orders, provided: (i) You provide advance written notice to the Wellness Center; and (ii) You pay all dues and other unpaid charges due at the time of the commencement of the suspension/freeze.  
  • Medical Suspension/Freeze Status.  You may apply for a “Medical Suspension/Freeze” of your membership for a period of two (2) months, not to exceed six (6) months, and no more often than once every calendar year provided: (i) I provide the Wellness Center with a note from a licensed physician before the commencement of the suspension/freeze; and (ii) I pay all dues and other unpaid charges due at the commencement of the suspension/freeze.  A note from a licensed physician will be required in order to reinstate my membership and to begin using the Wellness Center following my suspension/freeze. 
  • Snow Bird Suspension/Freeze Status.  You may apply for a “Snow Bird Suspension/Freeze” of my membership, if you are sixty (60) years or older, for a minimum period of one (1) month, up to a maximum of four (4) months, during the months of November through March, and no more often than once every calendar year by: (i) given advance written notice to the Wellness Center; and (ii) paying all dues and other unpaid charges due at the time of the commencement of the suspension/freeze.
During an approved suspension/freeze, I, and any secondary members listed on my account, may not use the Wellness Center.  The term of this Membership Contract will be extended by the period of the approved suspension/freeze.  No dues will accrue during the suspension/freeze.
 
**All freezes must be consecutive months in a calendar year.**
MEMBERSHIP STATUS CHANGE:
 
Membership Upgrades/Downgrades (adding/removing members, etc.) - must currently be done in person or via phone.
